Funding is provided by Circle of Care Foundation for a Care Manager through the Care Management Program at S+AGE™. S+AGE™ (Specialized Ambulatory Geriatric Evaluation) is an oupatient medical center that specializes in comprehensive geriatric assessment. At S+AGE™, older adults receive a thorough, inter-disciplinary evaluation and care plan to address any medical, cognitive, psychological, or social issues.

Family caregivers and non-family caregivers play an essential role in the ongoing health and well-being of S+AGE™ patients. Through assessment, education, counseling, and resource coordination, the Care Manager helps to integrate the caregivers into the overall plan of care so their concerns and needs are adequately addressed.
In addition to Care Management services, the S+AGE™ Care Manager provides specialized intervention when S+AGE™ patients are hospitalized. Modeled after the Care Transitions Intervention created by Dr. Eric Coleman, the Care Manager mobilizes additional assistance, visiting the patient in the hospital and at the next care site, whether that is the patient’s home or a skilled nursing facility. Throughout, the Care Manager is available as a resource, providing coaching, guidance, and resources as the primary caregiver navigates multiple healthcare systems.
For each S+AGE™ patient, the Circle of Care Foundation provides the S+AGE™ Health Essentials, a customizable guide that helps each patient organize their important health, insurance, and legal documentation.
